To request an appointment, please send an email to info @ drbailetti . com with all of the information below. Emails are usually answered within 48 hours.

  1. Name of client (and name of parent if under 18 years). If child is under 1 year, please specify name to appear on invoice.

  6. Comments or reason for visit

  7. Type of appointment requested:
    • Initial Naturopathic Visit (60-120 minutes)
    • Subsequent Naturopathic Visit (15-60 minutes)
    • Vaccination Consultation (60 minutes)
    • Vaccination Consultation and Support (120 minutes)
    • Subsequent Vaccination Consultation (15-30 minutes)

  8. Length of appointment requested:
    • 15 minutes ($40)
    • 30 minutes ($75)
    • 45 minutes ($100)
    • 60 minutes ($130)
    • 90 minutes ($180)
    • 120 minutes ($200)

  9. Preferred appointment setting:
    • Clinic
    • Phone (This is available for 1-hour Vaccination Consultations and some follow-up visits. Long distance calls are toll-free.)

  10. Person who referred you

Please note:

  • Email consultations are subject to a fee based on how much time is required to respond. A minimum fee of $40 for 0-15 minutes is charged. Email consultations are discouraged.
  • GST applies to all services.
  • Missed appointments and appointments cancelled with little notice (less than 24 hours) are subject to a $50 fee.
